Hartford Practice Initiatives

Hartford Practice initiatives will increase access to and dissemination of gerontological nursing best practices education to nurses and leaders of staff who care for older persons, expand degree options for upgrading the skill and preparation of nurse leaders in community-based and long term care settings, and build cooperative relationships with care agencies to enhance education, practice, and research.

Outreach Project

The quality of care for older Iowans in long-term care facilities is optimized when care is evidence based. The workshops focus on different care problems: urinary incontinence, skin care, and pain management. The workshops are designed for staff members form long-term care facilities, including RN’s, LPN’s and CNA’s. At the end of each session, participants will be able to:

  • discuss the problem of UI in LTC
  • describe the consequences of UI on the person and the employees and facility
  • identify treatable causes of UI
  • complete an incontinence diary
  • describe prevention strategies for UI
  • describe 2 interventions for UI that are successful in LTC
  • describe successful continence programs
  • describe strategies for implementation of a continence program in home facility
  • discuss the scope of the problem of pain management in LTC
  • identify ways to assess pain
  • complete accurately 2 pain assessment instruments
  • list ways to monitor response to pain measures
  • identify the role of each nursing team member in pain management
  • describe non-pharmacological comfort measures to assist in management of pain.
  • identify system barriers to pain management
  • apply the principles of pain management in home facilities
  • assess risk for development of pressure ulcers
  • complete accurately the Braden Scale
  • identify the difference between pressure and stasis ulcers and stage pressure ulcers.
  • understand how essential the team approach is in pressure-ulcer assessment and prevention, and the importance of each team member
  • apply the principles of assessment and prevention in home facilities
  • describe the preventative interventions and match them to risk factors

Each team will bring their facility’s policy on the topic and a case study of a person with the problem. An expert in the field will make a formal presentation, followed by a roundtable discussion where teams will present their cases and have opportunity for questions and information sharing. Teams will develop a plan of action to take back to their facilities. Each facility will receive a site visit by Project Coordinator between sessions to provide assistance and consultation as requested.

The cost for each facility which includes up to 5 participants, plus the Director of Nursing if she/ he wishes to participate is $250.Cost includes one copy per facility of the Gerontological Nursing Interventions Research Center protocols for each topic covered, speaker handouts and light refreshments. CEUs will be offered for licensed staff ($6 processing fee per individual per session). A full refund will be given if registration is canceled more then 7 days prior to the first seminar. There will be no refunds for cancellations fewer then 7days prior to first seminar.
